
Insurance Plans
Health plans which we accept:

An insurance policy is a contract between the patient and the insurance company. Lee OBGYN is not a party to this agreement. We are happy to file patients claims with the companies listed above. However having insurance is not a guarantee of coverage for services. It is responsibility of patients to communicate with their insurance company to determine their coverage/copay/deductible.

**NOTE: Blue Cross Blue Shield patients - if your policy begins with prefix BEG or PGX, you must contact your Primary Care Physician to obtain a referral prior to your appointment date. If we do not receive a referral by the date of your appointment, BCBS will not cover the visit and the patient will be responsible for the bill.

**LAB ORDERS - Lee OBGYN offers in house lab draws but sends labs out to outside labs for testing. If your insurance requires you to use a specific lab to process your orders, it is the patient's responsibility to inform your provider of this so that your lab tests can be sent to a lab that dictated by your specific insurance policy. Failure to do so may result in your insurance denying coverage and leaving the patient responsible for the charges.
